To qualify for a voucher, you MUST meet this criteria:
1. Polk county resident
2. provide an ID with a Polk County address
3. vouchers can only be used at Dixie day spay in Cleveland TN
4. There is an expiration date they must be used by
5. you will have a co-pay due to Dixie day spay at the time of the appointment. You make the appointment and if you’re a no show – you lose the deposit plus go on our “naughty list”
voucher is for a spay/neuter and rabies vaccination
Cats – your co-pay is $25
Dogs – your co-pay is $35
